What is a care needs assessment?

When creating a fully tailored and bespoke care package it is important to understand the level of care required to ensure you or your loved one are able to continue leading an independent and fulfilled life. Our elderly care needs assessment form asks a variety of questions to help us understand the level of care that may be required. The questions range from eating and drinking habits, mobility and orientation, dressing and hygiene, night-time support requirements, medical and specialist care needs and more. Following an analysis of your answers you will then receive a recommendation regarding the type of care that would be beneficial for you or your loved one. If you wish to discuss these care needs with your local representative they will be on hand to answer any questions, carry out a home visit and even create a fully tailored care plan to ensure the very best lifestyle. Promedica24’s bespoke home care plans

We believe that every person has the right to remain in their own home for as long as they choose, that is why all of our home care plans are 100% tailored to your exact needs and requirements. In order to ensure you receive the best care possible from one of our highly skilled and experienced carers, we will carry out an in-home care assessment before putting together your personalised home care plan. This will include spending time with you or your loved one to get to know what your hobbies and interests are. We will then match you to an experienced carer who shares similar interests to you to help you become as comfortable as possible with your new friend. We aim to put care in place within three to five days from the initial in-home care assessment and will carry out regular checks to ensure your level of care is still in line with your requirements.

What will the care needs assessment cover?

The care needs assessment will help you understand the level of care you currently require to continue living an independent lifestyle. Like our home care plan reviews, feel free to take this assessment regularly to keep track of the type of support that will help you continue living life to the full.

Eating and drinking habits
Whether you or your loved are able to eat and drink without assistance or prompting, through to requiring complete assistance or are at risk of choking.
Mobility and orientation
If you are fully mobile or require assistance from more than one person, need the assistance of a hoist or are confined to a bed.
Dressing and personal hygiene
Whether it would be beneficial to have assistance when it comes to dressing and maintaining personal hygiene.
Night-time support requirements
Not at all or will need night-time support several times a night
Medical needs
Has medication but is able to administer these independently or struggles taking medication and will require assistance
Communication
Has good communication skills or is withdrawn and non-communicative
